NEW YORK, Sept. 10 /PRNewswire-HISPANIC PR WIRE/ -- "Teaching Health, Changing Lives" is how Dr. Eliscer Guzman and The Mount Sinai Medical Center describe the Community Diabetes and Cardiovascular Care Program (CDCCP), a collaborative educational outreach program which was designed specifically for people from the vulnerable Latino community diagnosed with diabetes.
Twenty-five patients from The Mount Sinai Medical Center were randomly selected to participate in an 8-10 week course where they learned how to live and manage diabetes. On September 16th, all twenty-five students will gather to celebrate their graduation -- being the first class to complete the Community Diabetes and Cardiovascular Care Program.
Joining them will be select physicians from The Mount Sinai Medical Center and Dr. Eliscer Guzman who together provided a free, high quality educational program for patients from the Latino community.
