NEW YORK, Sept. 10 /PRNewswire-HISPANIC PR WIRE/ -- "Teaching Health, Changing Lives" is how Dr. Eliscer Guzman and The Mount Sinai Medical Center describe the Community Diabetes and Cardiovascular Care Program (CDCCP), a collaborative educational outreach program which was designed specifically for people from the vulnerable Latino community diagnosed with diabetes.
Twenty-five patients from The Mount Sinai Medical Center were randomly selected to participate in an 8-10 week course where they learned how to live and manage diabetes. On September 16th, all twenty-five students will gather to celebrate their graduation -- being the first class to complete the Community Diabetes and Cardiovascular Care Program.
Joining them will be select physicians from The Mount Sinai Medical Center and Dr. Eliscer Guzman who together provided a free, high quality educational program for patients from the Latino community. All of their hard work and dedication will be realized when each student receives their certificate.
Participate in this unprecedented event, meet the graduating patients and the medical professionals responsible for their education. The Community Diabetes and Cardiovascular Care Program has positively impacted the lives of diabetic patients from the underserved Latino community in New York.

This 10 week course teaches patients how to understand, cope, manage and live with diabetes.
